Affiliate Forms For Gravity Forms

Standard affiliate sign-up forms can be rigid. This plugin lets you build the registration form in Gravity Forms with its drag-and-drop editor, so you choose which fields appear and how the layout looks. You can use default Gravity Forms fields, along with three new AffiliateWP fields made for affiliate-specific information.

Only an email address is required. If the form has no username field, the plugin creates a WordPress username from the email address. If there is no password field, it generates a password and emails it to the affiliate. Each submission can be reviewed in your dashboard, which helps when you approve applications. The plugin requires Gravity Forms.
